type：类型
list：取值范围（list="empno, ename, hiredate"）（list="1，empno, 2，ename, 3，hiredate"）
required：yes  必填
description：yes  单选显示名称
restricted：yes 只可以选择，不可以填写
checkbox:复选框
prefix：前缀
suffix：后缀
