This problem can occur on any platform.
Symptoms
Checked for relevance on 20-JUL-2008.
Connections via dispatchers hang.
For each failed connection via dispatcher, the listener will log three errors in the listener log as shown.
TNS-12518: TNS:listener could not hand off client connection
TNS-12560: TNS:protocol adapter error
TNS-530: Protocol adapter error
For each failed connection attempt, the number of "refused connections", shown with lsnrctl services, will increment by 1.

Workaround:
Add the following line to the listener.ora
direct_handoff_ttc_=off
Example:
STARTUP_WAIT_TIME_LISTENER = 0
CONNECT_TIMEOUT_LISTENER = 60
TRACE_LEVEL_LISTENER = OFF
DIRECT_HANDOFF_TTC_LISTENER=off <=== add this parameter
