CAUSE
The similar problems were investigated in below bugs:

Unpublished Bug 34940829 - ALTER PROFILE ON CDB MAKES DB HANG WAIT ON 'DISK FILE OPERATIONS I/O'<='ROW CACHE LOCK' PATCH FOR BUG 33218259 IS INSTALLED

Unpublished Bug 34534493 - DATABASE CREATION HUNG.

 The callstack "_ZN11OracleHooks8readHookEiPvm" suggested that the problem is caused by third-party security software, eg IMPERVA

/opt/imperva/ragent/bin/ragentwd --daemon -r /opt/imperva/ragent -d /opt/imperva/ragent/etc -v /opt/imperva/ragent/var
 

SOLUTION
Stop third-party security software (eg imperva); (sometimes even uninstall it from the system)
