oracle的一ww,wwv_flow.accept error

I am getting an intermittent browser error when submitting a page in my application. The error is:

Not Found

The requested URL /pls/toolsdb/wwv_flow.accept was not found on this server.

This can happen either

(a) when I explicitly submit a page by clicking on a button, or

(b) when a select list is changed which submits upon the change.

There are four checkboxes on the page (among many other items). So far the error has only happened when all four are unchecked. I can avoid the error by checking at least one of them and then submitting. But this situation would have occurred many times in the past and there was never any error.

The application has not been modified in any way for the past few days. The intermittent error started occurring yesterday morning.

A look at the Apache error log gives this sort of message every time the browser error occurs:

Any insights that would help me solve this problem?
