CHANGES
- Upgraded to 11.5.10 from 11.5.9 and went to a RAC structure.
CAUSE
Dead process
Example:

Found dead process: spid=(123348), cpid=(27251), ORA pid=(25), manager=(0/1)
Could not contact Service Manager FNDSM_APPSERVER32_TTPROD. The TNS alias could not be located, the listener process on APPSERVER32 could not be contacted, or the listener failed to spawn the Service Manager process.
Could not contact Service Manager FNDSM_APPSERVER33_TTPROD. The TNS alias could not be located, the listener process on APPSERVER33 could not be contacted, or the listener failed to spawn the Service Manager process.
Could not contact Service Manager FNDSM_TTIRVOC1_TTPROD. The TNS alias could not be located, the listener process on TTIRVOC1 could not be contacted, or the listener failed to spawn the Service Manager process.
Found dead process: spid=(299528), cpid=(27261), Service Instance=(1253)
SOLUTION
1- As System Administrator, run the Concurrent Manager clean process (See Note: 134007.1 'Concurrent Processing - CMCLEAN.SQL - Non Destructive Script to Clean Concurrent Manager Tables' on running this process.)
2- Restart Concurrent Manager
3- Retest the issue
Datafix Steps for Current Failed Documents:
- Run wfretry first for the failed document
(Step Details Note 134960.1 for wfretry)
- If the Document remains stuck, then proceed with the following reset steps:
Reset Steps
- Download Patch 5549427
- Use one of the following included Document Type specific reset scripts for the desired Document
- The scripting will move the status of the Documents to either Incomplete or Requires Reapproval status
- poxrespo.sql (Purchase Order)
- poresrel.sql (Purchase Order Release)
- poresreq.sql (Purchase Requisition)
- Upon running the desired reset script, ensure the COMMIT is issued if there are no errors
- Query the reset document for editing and desired actioning (Approve, Cancel, Reject, Delete, etc.)
